Flu Vaccinations

Volume driven pricing varies depending on your total employee population.  Our pricing is based on a per person vaccine cost with no hidden, or additional,  fees.

Minimums do apply.  Your representative will go over the particulars for your specific location and number of employees at the time you schedule your program.

Pneumonia Vaccinations

$45.00 per person. 

Administration of this vaccine is not annually, but  between 5 and 10 years, depending on individual circumstances.

Welcome to the BHA Flu Vaccination Rebate Program!

Because you do not physically work at a location where your company is providing on site flu vaccinations*, you are eligible for a Flu Vaccination Rebate Program which has been pre-authorized by your employer.

This means that you can obtain a flu shot at the provider of your choice, in your area, and obtain reimbursement directly from us, BHA.

You will only be reimbursed for a flu shot, not for an office visit to obtain a flu shot. The maximum reimbursable amount for each flu shot is $20.00.

How does it work? Simply click on the button below and print out the form. COMPLETELY fill in every blank. Incomplete forms will be not be eligible. Return the form, AND your receipt, to us by fax, or email, as noted on the form. You will receive your check, within 60 days, to the business address you provide on the form.

*please note, if you work at a location where an on-site flu vaccination program is being provided, you are NOT eligible to participate in the rebate program. You must obtain your flu vaccination at your employer site.
